Teacher Fellowship Agreements
The following are the basic requirements for the CSME Teaching Fellowship. We also anticipate that each of you will contribute to the community in unique ways, and will be supportive of each other’s work.
Members
As a CSME Teaching Fellow I agree to meet the following requirements:
- Complete evaluations or written reflections as assigned to me
- Attend THREE CSME Teacher Community Meetings (Must stay for the entire program and attend the events as indicated below based on your content area)
Undergraduate fellows
As a CSME Teaching Fellow I agree to meet the following requirements:
- Meet for two individual meetings with CSME Teaching Program Director/CSME Faculty
- Meet with my academic advisor during the semester to confirm graduation date
- Apply for at least ONE scholarship, internship, or fellowship
- Do a minimum of 40 hours Early Field Experience (to be arranged w/ Program Director)
- Complete evaluations or written reflections as assigned to me
- Attend THREE CSME Teacher Community Meetings (Must stay for the entire program and attend the events as indicated below based on your content area)
Credential Fellows
As a CSME Teaching Fellow I agree to meet the following requirements:
- Agreement to host a CSME Teacher Fellow in your classroom
- Complete evaluations or written reflections as assigned to me
- Do a minimum of 3 hours of CSME Teacher Outreach (TBD w/ Program Director)
- Attend THREE CSME Teacher Community Meetings (Must stay for the entire program and attend the events as indicated below based on your content area)
Reimbursement Form
CSME will reimburse for various services related to your teacher preparation (CBEST, CSET, Credential Application to SFSU, Livescan, etc...). These funds are available on a first come, first served basis. Please download the form and submit complete reimbursmet packets to us in SCI 211.

Livescan Fingerprinting Instructions (On Campus at SFSU)
In order for the fellowship to fund your livescan, please follow these instructions:
1) Make an appointment with Campus Police at (415) 338-6043. When you are speaking with the representative, please tell her/him you are from the CSME Teacher Fellowship and that you are on our student list. This ensures that they bill the livescan to us (CSME) and that you do not have to pay for it up front.

2) Please be sure to bring the proper documentation and paperwork to the livescan appointment.
- In most cases, this will be the San Francisco Education Fund form which was provided to you during the San Francisco Education Fund training.
Please note without the proper documentation, your information will not be sent to the proper place (which would be the San Francisco Education Fund).
3) After you get fingerprinted, make sure you get some form of receipt or copy that you completed it.Live Scan Fingerprinting Locations (other than SFSU)
